My CCS Employment (Adjustments) Passport

If you require adjustments to be able to perform your role, the CCS Employment Passport is designed to support you with this.
The passport is for everyone, particularly if there is something you feel your manager should know about in order to support you in your work. For example, if you have carer responsibilities, a health condition, a disability, or anything related to your religion or culture.
The passport will:
- Make sure everyone needing to know is clear about what adjustments have been agreed
- Reduce the need to review adjustments if there is a change in line manager
- Form the basis of a revised passport if you change jobs or location and this affects the adjustments you require
- Provide you and your manager with a record of the agreed adjustments for regular discussion and review
